We are here to ensure that you and your family have the information and assistance needed to make informed decisions about your health and well-being.
1. Confidentiality: We take the privacy of patient’s health information very seriously, and have put necessary measures in place to ensure the confidentiality of the personal health information. Our office confidentiality policy is in accordance with the Health Information Act Guidelines and Practice. Staff are trained in confidentiality and security procedures during their orientation. Unless required by law, our office will not release any medical records without the patient or guardian’s consent.
2. Appointments:
· Specialty appointments are accepted based on Referrals. Please note that as we receive high volume of referrals, all the referrals received are triaged to ensure timely care.
· If you schedule an appointment, the scheduled time is reserved specifically for you/your child.
· Please bring the patient’s valid provincial health card number with you during each appointment. If the card is invalid, you will be billed directly for the visit.
· If your child does not have valid provincial health card coverage but has private insurance, we will bill you for the services and provide you with an official receipt. Use this receipt to claim reimbursement from your third-party insurer.
· Uninsured services are directly billed to patient or guardian.
· Notice for cancelling an appointment must be given 72 hours in advance of the appointment day/time, so that other patients from the waitlist could be scheduled in a timely manner. Missing the appointments without notifying timely would be billed to the patient/guardian.
3. Communication: Office email should be used for administrative purposes only and is handles by administrative staffs. Patients are encouraged to make phone calls or faxes, as emails are not considered secure. Any medical or clinical concerns should be addressed in person with your doctor. We use secure messaging system Medeo for patients and healthcare provider to communicate with each other.
